In this film an original technique, elaborated by the author (he has been using it since 1995) is presented. It is a technique of laparoscopic ligation of testicular veins for varicocele in children and teenagers. The peculiarity of this technique is exposure and ligation of the whole of the vascular vascicle of the left testicle with the careful dissection and preserving lymph flow, that is a safe prophilaxis against formation of postoperation testicle dropsy ( it is one of the most wide-spread complications of the traditional surgical methods of varicocele treatment). The 5 mm or 3 mm troacars are used.

In this film the technique of laparoscopic left adrenalectomy is presented. A 6 month-old infant has been operated on for big (about 6 cm in diameter) hormonally active tumour of the left adrenal gland. Three troacars have been used, a patient’s positioning is on the right side. The stages of tumour visualization are shown in brief (mobilization of spleen, pancreas tail), tumour exposure within the limits of healthy tissues, without disturbing capsule safety (as there is no possibility to exclude that tumour could be malignant), transection of vessels, total dissection of tumour that has been removed through the umbilical access later, using the endoscopic sac.

In this film the technique of laparoscopic left adrenalectomy is presented. A 3 year-old child has been operated on for rare kind of hormonally active tumour of the left adrenal gland-aldosteroma. Three troacars have been used, a patient’s position is on the right side. The stages of visualization of tumour are shown briefly (spleen mobilization and that of pancreas tail), the adrenal gland with tumour within the limits of healthy tissues, transection of vessels of the adrenal gland, removal of the adrenal gland, using endoscopic sac.

The operation is performed for benign tumour of the adrenal gland (the diameter is 6 cm), it is located in the lateral pedicle. In this film the technique of organopreserving operation, dissection of tissues by endoscopic scissors with the exposure of the inferior vena cava and adrenal vein is presented. Transection of the vascular structures and tissue of the adrenal gland is performed by a 5 mm Thunderbeat Olympus instrument. The operation is performed fast and without blood loss. Operation duration is 25 minutes. Additional hemostasis is done by PerClot system (Italy).

The operation is performed for benign tumour of the adrenal gland, having the diameter 12 cm. In this film the technique of organopreserving operation, dissection of tissues with the help of endoscopic scissors and exposure of the inferior vena cava and adrenal vein is presented. Transection of the vascular structures and tissue of the adrenal gland is performed by a 5 mm LigaSure MEDTRONIC COVIDIEN instrument. The use of modern ligating systems gives a possibility not to perform an additional hemostasis of the stump of the adrenal gland and not to leave drainage in the abdominal cavity.
