In this video the technique of laparoscopic splenectomy by means of a 5 mm Harmonic Scalpel Ethicon instrument is demonstrated. Operation is performed for idiopathic thrombocytopenic purpura (ITP). At first splenic artery is exposed and temporarily clipped. It is done to prevent the risk of bleeding during handling of vascular pedicle of spleen. Then, using Endo GIA MEDTRONIC COVIDIEN apparatus (with a 60 mm white casette), splenic vessels are stitched and transected. Preparation is placed into a special plastic container MEDTRONIC COVIDIEN, that is removed through the expanded access at the end of operation.

Operation is performed for benign tumour of the spleen, having the size 12 cm. In this film the technique of a careful dissection, especially that of renalsplenic ligaments by means of ultrasonic scissors and a 10 mm LigaSure MEDTRONIC COVIDIEN instrument is presented. The use of the energetic platform LigaSue MEDTRONIC COVIDIEN gives a possibility safely to “weld” splenic arteries and veins without use of a stitching apparatus and clipping of vascular structures. Operation duration is 20 minutes. There is no necessity to leave drainage after the operation. The removal of the preparation is done in a special plastic container.
