Laparoscopic resection of sigmoid colon, using the single access (S.I.L.S. Port COVIDIEN)

Laparoscopic resection of sigmoid colon, using the single access (S.I.L.S. Port COVIDIEN).
Professor Puchkov K.V. is performing an operation (2009).

In this video the technique of laparoscopic resection of the sigmoid colon, using the single port (SILS port COVIDIEN) for villous tumour-4 cm- on the wide base with displasia. Port was placed in the umbilical. The technique of dissection of mesentery of the sigmoid colon with a 5 mm instrument (LigaSure MEDTRONIOC COVIDIEN) and a monopolar electrode of Karl Storz Company is demonstrated. In case of using the single port device, we perform a transcutaneous fixing stitches on the anterior wall of colon. Tumour area is marked from the side of serous membrane under the guidance of intraoperation colonoscopy. The marked area of the sigmoid colon is transected in the transverse direction by an endoscopic stitching 60 mm linear instrument Endo GIA MEDTRONIC COVIDIEN. The specimen was removed from the abdominal cavity alongside with the single port. Then proximal part was extracorporeally dissected with the tumour, and the head of a 31 mm endoscopic stitching circular device MEDTRONIC COVIDIEN was inserted into this part. The colon was immersed into the abdominal cavity, and again the single port was introduced there. Mechanic anastomosis “end-to-end” was formed under the guidance of a laparoscope. Operation duration was about 50 minutes.
