In this video the technique of greater omentum extirpation and peritoneumectomy with uterine cervix extirpation is presented. A 69 year-old patient is being operated on for ovarian cancer, stage III C, after the three courses of neoadjuvant chemotherapy. The operation starts with the greater omentum extirpation that is performed with the help of ultrasonic scissors, monopolar and bipolar energy. Peritoneumectomy is performed at the next stage. Then extirpation of uterine cervix is performed. The author is commenting on all details of all stages of his operation.

A 28 year-old patient is operated on for infiltrating endometriosis, invading to the wall of rectum and urinary bladder. According to cystoscopy data on the mucous layer of the urinary bladder there is tumour-like excrescence of a black colour-2-3 cm-with the fluid component inside. In this video the technique of endometrioid infiltrate exposure from the surrounding tissues by means of a 5 mm monopolar electrode is demonstrated. An affected area of the urinary bladder is dissected with the help of 5 mm MEDTRONIC-COVIDIEN ultrasonic scissors. Then it is stitched by “Polysorb” 3-0 and “Monocril” 3-0 thread. Before laparoscopy beginning stent is inserted into the lumen of the left ureter. One should take control of the stitched wound-whether it is hermetic or not.

Laparoscopic splenectomy

Operation is performed for benign tumour of the spleen, having the size 12 cm. In this film the technique of a careful dissection, especially that of renalsplenic ligaments by means of ultrasonic scissors and a 10 mm LigaSure MEDTRONIC COVIDIEN instrument is presented. The use of the energetic platform LigaSue MEDTRONIC COVIDIEN gives a possibility safely to “weld” splenic arteries and veins without use of a stitching apparatus and clipping of vascular structures. Operation duration is 20 minutes. There is no necessity to leave drainage after the operation. The removal of the preparation is done in a special plastic container.
