The patient is 42, has infiltrating endometriosis, invading to the wall of the urinary bladder. In this video the technique of exposure of endometrioid infiltrate by means of bipolar forceps and endoscopic scissors, with the preliminary exposure of the right ureter is demonstrated. During cystoscopy invasion of endometrioid infiltrate to the mucous layer has been detected. Then resection of the affected area of the urinary bladder is done and atraumatic interrupted Z-suture is placed, using “Monocril” 2-0 thread. It is required to take control of suture-whether it is hermetic or not, and to place additional suture in the area of the urinary bladder.

The patient is with infiltrating endometriosis, invading to the vaginal wall without involvement of rectum into the process. In this video the technique of exposure of the endometrioid infiltrate by means of bipolar forceps and endoscopic scissors without the preliminary exposure of the ureter is presented. And the technique of resection of the posterior vaginal wall with further stitching, using “Monocril” 1-0 thread, is demonstrated, as well.

In this video the technique of laparoscopic myomectomy with the temporary occlusion of the arterial stream (uterine and ovarian arteries) is demonstrated. The patient is operated on for the intramural myoma-6 cm on the anterior and 2 cm on the posterior wall of the uterus. At the first stage the pelvic abdomen is opened , and the uterine artery is exposed behind the ureter, the artery is temporarily clipped. Ovarian arteries are clipped in the area of infundibulopelvic ligaments. It gives a possibility to decrease blood loss during the operation. Then the muscular tissue is dissected up to the myomatous node with the help of a monopolar electrode, and myoma is enucleated from the surrounding tissues by means of clips without opening the uterine cavity. The wound is stitched by the interrupted suture, using “Monocril” thread, made of the filament absorbable polydioxanone material. Then clips are removed from the arteries, and blood circulation is restored in the uterus. The myomatous node is removed from the abdominal cavity with the help of Rotocut G1 electromechanical morcellation (Karl Storz Company).

n this video the technique of laparoscopic supracervical hysterectomy and classic promontofixation with the use of combination of monopolar scissors MEDTRONIC COIVIDIEN and bipolar forceps (Karl Storz Company) is demonstrated. Excision of the body of uterus from its cervix is done by means of the original monopolar loop SupraLoop Karl Storz. Promontofixation is performed with the help of a special mesh implant with the split shreds. Uterus morcellation is done mechanically in a special plastic container. The video presents all detailed comments on of all the stages of the operation.
